Its core focus includes initiatives for hard-to-reach populations through services like outreach clinics, training of community midwives to provide low-cost health services in remote areas, screening for non-communicable diseases, and digital health solutions.
Job Summary: The Data Entry Operator is responsible for accurate and timely entry of hospital and patient-related data into registers, HMIS/DHIS systems, and electronic databases. The position ensures proper record management, reporting support, and data quality to assist hospital administration in planning, monitoring, and decision-making.
Aga Khan Health Service, Pakistan, is looking for a competent Data Entry Operator based at RHC Shagram, Upper-Chitral in Chitral Region.
Key Responsibilities:
A. Data Entry and Record Management
- Enter patient and service delivery data into HMIS and DHIS and hospital information systems accurately.
- Maintain OPD, IPD, emergency, laboratory, pharmacy, and other departmental records as assigned.
- Update daily patient attendance, admissions, discharges, referrals, and treatment records.
- Ensure completeness and correctness of entered data by cross-checking registers and forms.
B. HMIS Reporting and Documentation
- Compile daily, weekly, and monthly reports as per HMIS & DHIS requirements.
- Support preparation of performance reports for hospital management and donors
- Maintain proper filing and storage of HMIS forms, registers, and reports.
- Assist in data consolidation from different departments for final submission.
C. Data Quality Assurance
- Verify accuracy of data received from different hospital departments.
- Identify missing or inconsistent information and coordinate with concerned staff for correction.
- Ensure data validation and follow data quality standards.
- Maintain data confidentiality and ensure safe handling of sensitive patient information.
D. Database and System Management
- Maintain electronic databases and ensure data is updated regularly.
- Backup hospital data and maintain proper record security.
- Ensure smooth functioning of computers, printers, and data systems; report technical issues promptly.
- Support installation and use of new data software and reporting tools when required.
E. Coordination and Communication
- Coordinate with doctors, nurses, paramedics, and department heads for timely collection of data.
- Provide support during audits, monitoring visits, and supervision by management or health department.
- Assist hospital management in retrieving data and preparing presentations or summaries.
F. Confidentiality and Professional Conduct
- Maintain strict confidentiality of patient and hospital information.
- Ensure secure handling of documents and restrict unauthorized access.
- Follow hospital policies, code of conduct, and safeguarding guidelines.
- Perform other duties assigned by supervisor related to record and data management.
G. Key Performance Indicators (KPIs)
- Accuracy and completeness of entered data.
- Timely submission of HMIS and hospital reports.
- Reduction in data errors and missing entries.
- Proper filing and retrieval of records.
- Support provided during audits and monitoring visits.
- Compliance with confidentiality and data protection standards
Qualifications & Experience:
- Intermediate/Bachelor degree (preferred) in IT, Computer Science, Statistics, or relevant field.
- Minimum 1–2 years of relevant experience in hospital/health sector data entry preferred.
- Experience working with HMIS or DHIS2 will be an advantage.
