Rehabilitation Field Intern (with Focus on Referrals and Patient Data)

General mission

Seeking a Rehabilitation Field Intern to join our ReLAB-HS team. This is a dynamic field-based internship designed for individuals eager to gain hands-on experience in the rehabilitation of patients at Primary Health Centers (PHC). The intern will assist in managing patient referrals, maintaining and updating patient data, and supporting healthcare professionals in providing high-quality rehabilitation care at the District Swat selected health facilities in Tehsil Barikot.This role requires close coordination with the District Health Office (DHO) and involves working in different health facilities in Tehsil Barikot at the PHC level, depending on the needs of the community. The position provides an excellent opportunity to engage directly in patient care and rehabilitation interventions while gaining exposure to field-based healthcare settings.

 

Missions / responsibilities

SPECIFIC OBJECTIVES:

  • Referrals Management:
    • Assist in receiving and reviewing patient referrals from the community.
    • Coordinate with physicians, therapists, and other departments to ensure timely follow-up on patient referrals.
    • Ensure accurate and up-to-date patient referral data is entered into the system.
    • Facilitate the scheduling of initial and follow-up consultations for referred patients.
  • Patient Data Management:
    • Maintain and update, ensuring they are complete, accurate, and secure.
    • Support the team in the analysis and reporting of patient progress based on treatment data.
    • Monitor and ensure that patient data confidentiality is maintained in compliance with healthcare privacy regulations.
  • Rehabilitation Support:
    • Assist in carrying out physical, and functional assessments as directed by licensed therapists.
    • Participate in the preparation and organization of therapy sessions, ensuring patients have necessary equipment and materials if needed.
    • Observe and assist in exercises, mobility training, and other therapeutic activities.
    • Document patient responses to therapy and report changes or concerns to the supervising therapist.

Assistive Technology (AT) Hub Management:

  • Assist in managing the AT Hub, which includes coordinating the availability, fitting, and maintenance of assistive devices for patients in need.
  • Support the medical officers in the assessment of patient needs for assistive technology devices such as mobility aids, communication devices, or adaptive tools.
  • Help with the documentation and tracking of assistive technology devices issued to patients, ensuring proper usage and follow-up.
  • Collaborate with healthcare professionals to ensure that assistive technology aligns with the rehabilitation goals and meets the functional needs of patients.
  • Monitor the effectiveness of assistive devices, gather patient feedback, and assist in adjusting devices as needed.

Collaboration with Health Teams:

  • Work closely with the District Health Office (DHO) and local healthcare teams to ensure that rehabilitation services are provided in a coordinated and effective manner.
  • Collaborate with rehabilitation specialists, healthcare providers, and community health workers to ensure a holistic approach to patient care.
  • Participate in meetings and discussions regarding patient progress and treatment adjustments.

Skills/Education:

  • Graduate of a program in Physical Therapy, Occupational Therapy, or related healthcare fields.
  • Basic understanding of rehabilitation principles and therapeutic interventions.
  • Strong attention to detail and ability to maintain accurate patient data.
  • Excellent written and verbal communication skills.
  • Familiarity with medical data management systems is a plus.
  • Ability to work effectively within a team and take initiative when needed.
  • Ability to communicate in local language
